Nomination of Beneficiary Form
The nomination of beneficiary form is used for the nomination of dependant(s) who in the event of death will become the recipient(s) of a percentage of the assets in the super fund.
This is a one page document. Up to four dependants can be named. Alternatively one legal representative can be named to distribute the assets in accordance with the Will or relevant Intestacy Laws.
